Labgram #44/Valgram #66 - New Requirement for End of Evaluation Survey (F8004)All, Please be advised that vendors are now required to submit an End of Evaluation Survey for all NIAP evaluations completed from this date forward in order to be posted to the Validated Products List (VPL). This form, (F8004) “NIAP CCEVS End of Evaluation Survey,” can be found on our web site at: http://www.niap-ccevs.org/cc-scheme/forms/ The survey may be submitted through the CCTL as part of the final package OR faxed directly to CCEVS at 410-854-6615.
